The following elements are responsible to notify or participate, upon request, in accident investigation or any other duties requested by DGCAA as a part of the duties and responsibilities or as an assignment:

 

  • Air Traffic Control (ATC) Responsibilities:
The primary role of ATC in case of aircraft accident/incident is to; notify the DGCAA, AAIU, all appropriate JCAA directorates and other involved organizations.
ATC should:
 
  1. Advise DGCAA and alert Search and Rescue, immediately after receipt of initial notification, ATC will also notify AAIU.
  2. Notification procedures concerning accident/incident are to be conducted as prescribed by this manual in chapter 5 (Air Traffic and Other Initial Notification)
  3. Advise the appropriate JCAA directorates of the results of Search and Rescue.
  4. Report Air Traffic incidents including Aircraft Proximity (AIRPROX) and Near Mid Air Collisions (NMACs), Pilot Deviation, Vehicle or Pedestrian Deviations and other occurrences.
  5. Submit any information requested by AAIU immediately.
  6. Supply AAIU with any Air Traffic Controller, if requested, for the purpose of investigation. The concerned ATC personnel should be under AAIU IIC direction until released.
  7. Take procedures in order to comply with any requested corrective actions recommended by AAIU IIC as prescribed by. Prompt Action Request Form (Form No. 31-6).
  • Flight Safety Directorate (FSD) Responsibilities:
    In case of accident / incident occurrence, FSD is responsible to:
  1. Notify AAIU about any accident/incident as prescribed by this manual,chapter 5 (Air Traffic and Other Initial Notification).
  2. Ensure that all facts and circumstances leading to the accident are recorded and evaluated, and action is taken to prevent similar accidents.
  3. Promulgate and enforce Jordan Civil Aviation Regulations (JCARs) for certification civil aircraft airworthiness, for certification airmen and operators for competency. This responsibility includes the continued “surveillance” of the airworthiness of aircraft and competency of airmen, air agencies, commercial operators, and air carriers, and the safety of airports.
  4. Conduct investigations and submit factual reports of the investigations to AAIU on accidents delegated by AAIU to FSD.The delegation should be given as per this manual, chapter 6 (Accident Investigation).
  5. When AAIU committee of investigation includes one or more members from FSD, the investigation should be conducted as per this manual, chapter 6 (Accident Investigation).
  6. Manages a system for FSD responses to AAIU safety recommendations.
  7. Identifies safety and corrective action issues that arise from accident and incident investigations which will reduce the likelihood of recurrence and enhance air safety. 
  8. Conducts analyses of air carrier and general aviation accident and incident data to identify trends and safety deficiencies.
  9. Develops and monitors a system for disseminating factual information identified as a result of accident and incident investigations. 
  10. Manages the Corrective Action Program.
  11. Upon receiving safety recommendation, shall address them with the thoroughness necessary to respond to the deficiency or to propose alternate actions. In no case will a recommendation be ignored.
  12. In case of accident or incident prevention recommendations related to deficiencies of design, operations, or maintenance practices or of established standards, procedures, or policies shall be reviewed as described below:
  1. FSD inspector shall prepare an internal report and submit it to his / her Director. The report shall contain a brief description of the accident or incident and the deficient areas. Sufficient details and / or substantiating information should be included so that the reader understands the development of the recommendations. This description will be followed by safety recommendations.
  2. A separate recommendation should be written for each issue. If possible, the recommendation should identify how this will resolve the identified safety problem.
  3. The report is to be reviewed by the concerned division chiefs and any amendments or comments should be included
  4. After completion of the report, FSD Director shall review the report designated persons to put a plan on how the recommendation will be implemented.
  5. As the recommendation of AAIU report and FSD inspector’s internal report demand, FSD may re-evaluate the competency of certificated airman, air carrier, commercial operator, and airport or air agency.
  6. VI. All correspondences on corrective actions that were taken due to accident investigation findings should contain the AAIU accident investigation report number. 
  • Air Navigation Services Safety and Standard Directorate (ANSSD) Responsibilities: 
  1. Notify AAIU about any accident/incident as soon as possible. 
  2. Ensure the implementation of Jordan Civil Aviation Regulations (JCARs) for certification or approval for Air Navigation System. 
  3. Develops and monitors a system for disseminating factual information identified as a result of accident and incident investigations.
  4. Address received safety recommendation with the thoroughness necessary to respond to the deficiency or to propose alternate actions. In no case will a recommendation be ignored. 
  • Airport Safety and Standard Directorate:
  1. Notify AAIU about any accident/incident as soon as possible.
  2. Ensure the implementation of Jordan Civil Aviation Regulations (JCARs) for certification or approval for Airport System.
  3. Develops and monitors a system for disseminating factual information identified as a result of accident and incident investigations.
  4. Address received safety recommendation with the thoroughness necessary to respond to the deficiency or to propose alternate actions. In no case will a recommendation be ignored.
     
  • Aircraft Accident Investigation Unit (AAIU):
    AAIU duties and responsibilities concerning aircraft accident / incident are:  
  1. Conduct investigations, as required, in major air carrier accidents or incidents, air taxi, or general aviation accidents or incidents which reflect a lack of safety consciousness; and accidents and incidents that are catastrophic or involve recurring safety problems.
  2. Conducts investigations of AIRPROX (Aircraft Proximity), NMACs (Near Mid Air Collisions), operational errors, pilot deviations, runway incursions; vehicle and pedestrian deviations. 
  3. Develops and manages accident and incident reporting programs and furnishes accident and incident information to other JCAA elements.
  4. Assesses techniques and methods of accident and incident investigation; prescribes accident and incident investigation policies, practices, and procedures. 
  5. Maintains a duty roster of the 24-hours Office of Accident Investigation Unit duty officer for purposes of coordination and notification.
  • Civil Aviation Security Directorate Responsibilities:
  1. Notification of accidents and incidents involving possible illegal activity such as drugs, stolen aircraft, aircraft false markings, or accidents and incidents having a security, hazardous materials and/or, atmospheric / radiological contamination interest will normally be received by Civil Aviation Security Directorate. However, information might also come directly from other sources such as AAIU, law enforcement parties, the military, or the public.
  2.  Civil Aviation Security Directorate is designated as the prime focal point on the exchange of information on accidents and incidents involving aviation security, hazardous materials, and incidents involving or suspected of involving explosion and explosive devices (bombs).
  3. Director Civil Aviation Security shall make arrangements for receiving immediate notification of accidents and incidents with security implications.
  4. Upon receipt of accident or incident information involving possible illegal activity or having security or hazardous materials or atmospheric / radiological implications, Security Directorate will provide reports to the AAIU IIC.
  5. Security Directorate will provide specialists in the field of technical investigation in accidents/incidents involving hazardous materials, explosive or incendiary devices (bombings). Those specialists shall report to the AAIU IIC.
  • Aviation Medicine Responsibilities:
  1. Aviation Medicine shall conduct medical investigations of fatal and nonfatal aircraft accidents to determine the relationship of medical, toxicological, and other human factors to accident cause, and / or aircraft design, and to crash injury and survival.
  2. Medical investigations of aircraft accidents shall be related to clearly defined goals and objectives. Autopsy and toxicologica1 information shall be obtained in fatal accidents and compiled with information in airmen’s medical records to identify possible contributions of medical conditions and toxic agents to accidents.
  3. Develop all policies and coordinate all functions of the aviation medical program in aircraft accident investigations.
  4. Periodically identifying requirements in the medical investigation of aircraft accidents; planning, reviewing, and recommending projects that will fulfill requirements; and to review the status of assigned investigators.
  5. Participate in air carrier aircraft accident and incident investigations, as required.
  6. Conduct special investigations of aircraft accidents at the request of the AAIU.
  • Airport Management Responsibilities:
  1. Provide Airports specialists in different fields upon request of AAIU IIC.
  2. Special emphasis will be given to those items required and contained in the Airport Operations Manual. Investigations should include an analysis of the self-inspection reports prepared by airport personnel to determine if a deficiency that may possibly relate to the accident was reported, and what actions were taken to correct the deficiency. 
  3. If, during the investigation, possible violations of JCAR have become evident, airport management shall take its corrective action immediately.
  4. All Airports representatives will report to the AAIU IIC and may be assigned to a working group when their expertise is required.
  • Legal Advisor Responsibilities:
    The Legal Advisor is responsible for all legal services required to determine the proper performance of JCAA functions. Specifically, the legal advisor shall:
  1. Report to the AAIU IIC upon arrival at the accident or incident scene. If not equipped to proceed to the scene, the advisor will establish a working arrangement with the AAIU IIC, where by the advisor will be kept informed of the progress and development of the investigation.
  2. Provide appropriate legal review of all evidences acquired by CAA during the investigation. 
  3. Determine if any additional evidences should be obtained concerning the legal aspects of the investigation and request that evidence. This shall not affect the responsibility of the AAIU IIC to ensure that all available relevant evidence is obtained. The legal advisor should not binder the normal investigative process.
  4. Provide legal clearance of all statements, documents, and related evidence presented by CAA during the investigation. 
  5. Brief AAIU personnel on their conduct and responsibilities in the presentation of evidence at formal or informal interviews, hearings, or depositions.
  6. Provide any CAA legal material that might be required during an investigation, such as opinions on the effect of statutory provisions or related memoranda.
  7. Handling judicial investigation if enforcement action is to be taken as recommendation from the AAIU.
  8. Continuing the process to transfer the investigation to the Public Attorney if there is a criminal act violation.
  • The Involved Aircraft Operator/s Responsibilities:
    In case of accident/incident, the Operator/s is/are responsible to:
  1. Notify JCAA / AAIU about any accident / incident by following the procedures that comply with Jordan Civil Aviation Law, Regulations and this manual, chapter 5 (Air Traffic And Other Initial Notification).
  2. Notify JCAA / AAIU about any dangerous good aboard the involved aircraft.
  3. Supply the AAIU IIC with any requested documents, records, manuals, technical publications and any other materials and by the most expeditious means.
  4. Make sure that the Flight Data Recorder and Cockpit Voice Recorder are not interrupted by any person until AAIU Investigators arrive to the scene and release the removal under their supervision.
  5. Avail Technical Advisor to AAIU IIC, if requested. The Advisor will be under the directions of AAIU IIC until released. He / She will not participate in the investigation.
  6. Avail any requested personnel to AAIU for the purpose of investigation.
  7. Avail a free access for AAIU Investigator to any lab, shop, library or section and provide any requested equipment, test unit or research facility to be used for any aircraft system or component testing.
  8. Remove all the aircraft wreckage, in case of accident, after being released by AAIU IIC.
  9. Comply with any Prompt Action Request Form (Form No. 3 1-6) by the most expeditious means.
  10. P1an for procedures to apply all the AAIU Final Report recommendations and submit a feedback notification regarding the proposed remedy actions and the related Quality Assurance Program.
  11. The participation of the Operator/s in the accident investigation, reporting or news release shall not exceed that prescribed by the Accepted Operator Aircraft Accident Response Manual or similar manuals.
